Memory Care Connections

Memory Care Connections   

Memory Care Connections provides information and support to people caring for loved-ones with dementia. Live-in care partners and their family members can take part in Memory Care Connections. When family care partners receive the support and services they need, the person with dementia also benefits. As a result of the program, many care partners have been able to postpone or avoid placing their spouses in residential care.

People who’ve participated in the program report:
fewer symptoms of depression and stress
better physical health—compared with those who did not participate
increased satisfaction with the emotional and practical support received
increased closeness to their families. 

As participants in the program, you and other family members meet with an ADRC staff member who will:
Assess your situation and discuss your concerns
Help you understand Alzheimer’s disease and related dementias
Help you learn how the dementia progresses over time
Work with you to develop a support plan
Discuss strategies, provide coping tools and problem solving skills that will help reduce stress
Assist you in connecting with community resources and supportive services
Be just a phone call away.
